Chronic Conditions & Medicare

In order to qualify for one of these plans in South Carolina, you must have one or more of the following conditions:

1. Diabetes Mellitus

2. Chronic Heart Failure

3. A Cardiovascular Disorder

You must also have Medicare Parts A&B and live in a plan’s service area. These plans cover the same Medicare services that all Medicare Advantage plans must cover. However, costly maintenance services, such as glucose monitors and extended hospitals stays are included.

Because your health condition can change, SNP plan members can qualify for a Special Enrollment Period. This allows you to select a new plan, outside of the Annual Enrollment Period, based on your current health status.

**There are also SNP plans available for people who live in a nursing home (I-SNP) and those with both Medicaid and Medicare (D-SNP).
